In 2013, NHS North West London, the largest Service Transformation Partnership (STP) in the UK, engaged TrueCue to help them achieve their vision to improve the engagement with their partners across the range of care settings.
With over 400 GP practices, 10 hospitals and 4 mental health and community health trusts, one critical organisational challenge in achieving this, was that these partners all operated their own transactional systems collecting data in silos.
There was a collective desire to consolidate the data across the different partners to enable a single patient view, informing multiple stakeholders of the complete patient pathway, to better understand the needs of the patient and, ultimately, improve the integrated care for that patient.
